In un preciso istante, tutti i bambini della Terra restano paralizzati e pronunciano all'unisono le parole «stiamo arrivando».
Il governo inglese è a conoscenza di una razza aliena chiamata "456" che nel 1965, in cambio della cura per un nuovo ceppo di pandemia influenzale, richiese la consegna di dodici bambini. La stessa razza aliena si è ora messa nuovamente in contatto con la Terra, richiedendone il sacrificio di milioni.
A Jack Harkness, che fu uno dei soldati ad occuparsi dello scambio nel 1965, viene impiantata una bomba nello stomaco e insieme a lui esplode l'intera sede dell'Istituto Torchwood. Gwen, che ha da poco scoperto di essere incinta, si mette in fuga insieme al marito Rhys Williams. Dopo l'esplosione, il corpo di Jack si rigenera fino a tornare allo stato normale.
Nel frattempo, i capi di Stato delle maggiori nazioni del mondo si riuniscono a Londra, dove la razza aliena effettua i primi contatti diretti.
Jack scopre che un particolare segnale audio sarebbe in grado di uccidere gli alieni, ma per farlo funzionare è necessario utilizzare un bambino come punto di trasmissione; Harkness all'ultimo minuto decide di sacrificare suo nipote Steven, unico bambino in quel momento disponibile, per evitare che milioni di bambini siano venduti agli extraterrestri.
La minaccia aliena è scongiurata, ma Ianto perde la vita; dopo ciò che il capitano è stato costretto a fare alla sua famiglia, decide di lasciare Cardiff e rifarsi una vita altrove.
